Fingerprint Request Form

Please fill out this form to request to schedule an appointment to be fingerprinted. All fields are required. If you have any questions regarding this form, please email parking@iastate.edu.

Please request your top 3 preferred times between 8:00am-3:00pm Monday-Friday (holiday hours may vary). We will contact you with appointment time with in 3-5 business days.

ISU Fingerprinting Service is $30. Some fingerprints may have issues during processing and need to reprinted. Reprints are free with proof of receipt.

Alternatively, Fingerprinting Services are offered at the Story County Courthouse for a fee of $20. You may call 515-382-7464 to request an appointment.
